Shoulder pain is a common musculoskeletal problem. It ranks third after back and neck pain in the general population. One of the main reasons for the frequent occurrence of shoulder pain is that the shoulder joint is the most mobile and complex joint in the human body. The shoulder joint is surrounded by soft tissues such as the capsule, ligaments, tendons, bursa, and muscles. These soft tissues mainly contribute to the stability of the joint. As a result, soft tissue lesions in the shoulder are common.
The most common shoulder pain is due to issues within the shoulder's own structures. The most frequent conditions include impingement syndrome affecting the shoulder tendons and rotator cuff syndrome. Frozen shoulder, tendinitis (bicipital tendinitis), calcific tendinitis caused by calcium accumulation in the tendons, osteoarthritis (degenerative joint disease), shoulder instability, or subluxation (partial dislocation) are among the most common problems. Myofascial pain syndrome, which causes strain-related pain in the muscles around the shoulder, is also common. Shoulder pain can also occur due to nerve damage in the muscles around the shoulder, often leading to weakness or fatigue in the shoulder. Inflammatory rheumatic diseases often affect the shoulder (rheumatoid arthritis, spondyloarthropathy, polymyalgia rheumatica). Additionally, tumors or infections originating from the bones and soft tissues in the shoulder area can cause pain.
Less common causes of shoulder pain are related to issues outside of the shoulder itself. Cancers in the upper part of the lungs, liver, stomach, gallbladder, spleen problems, and heart conditions can also manifest as shoulder pain. It is important to note that shoulder pain and neck pain may often be confused, especially when neck disc herniation radiates pain to the shoulder.
For most pathologies causing shoulder pain, the primary treatment involves physical therapy and rehabilitation. In cases that require surgery, physical therapy before and after surgery is also important. For long-term management and prevention of recurrence of shoulder issues, it is essential to protect the joint, avoid strenuous activities, and engage in regular exercise.
